Brian is a former division 1 lacrosse player who suffered an ankle breakage in his left medial malleolus and as a result has two screws, with limited range of motion within inversion, eversion and dorsiflexion.
As a consequence of the injury and hardware, his ankle is not very “train-able”. He has a hard time differentiating pain/poor quality stimulus from high quality stimulus and the neurology of the tissue is fairly turned off: he has a difficult time finding a potent passive stretch, the lengthened isometric isn’t easy to initiate and it’s very hard for him to contract shortened tissue (where the break was), due to the lateral ankle tissue limiting his range.
Although we still have a difficult time navigating the tendencies his ankle has, it is significantly easier for him to “find” the correct tissue engagement: he’s more proprioceptive to finding the passive stretch and both the lengthened and shortened isometric contractions (PAILs/RAILs to increase joint capsule space), and the tissues responsiveness to the introduced stimuli has improved so his ankle training is considerably more productive than it was when I first had him doing this stuff six or so months ago.
