In this video, I present a compelling case: a 31-year-old female who developed severe facial sagging and double chin after previous square jaw surgery (mandibular angle resection with long-curve technique, 2020) and zygomatic reduction (2018).

▶ PATIENT HISTORY
• After jaw surgery, she had jaw reconstruction with bone cement
• Bone cement caused excessive jaw angle lengthening → masseter muscle indentation
• Bone cement was removed → chin projection lost, severe facial sagging, double chin

▶ PATIENT REQUESTS
• Increase chin projection (but NOT chin length)
• Minimize jaw angle lengthening during reconstruction (to prevent masseter indentation)
• Improve facial sagging through jaw reconstruction

▶ SURGICAL PLAN & RESULTS
Full mandible reconstruction using custom PCL (polycaprolactone) scaffold + autologous bone graft with 3D digital planning:
• Custom-designed anterior and posterior guides (nerve protection)
• Minimal jaw angle lengthening + body volume augmentation + chin advancement effect
• 6-month post-op CT: Excellent cortical and cancellous bone regeneration
• Dramatic improvement in facial sagging and double chin WITHOUT facelift
• Natural jawline contour restored

This case demonstrates that jaw reconstruction alone can effectively reverse facial sagging caused by over-resection of the mandible — without the need for a facelift.
