Can you fix protruding teeth AND an underbite without changing the bite? Yes — with rotational double jaw surgery. In this video, I walk you through a real patient case where I used clockwise rotation of the maxillomandibular complex (4.2mm) to correct severely proclined upper incisors and mild mandibular prognathism, all while preserving the existing occlusion.
This 29-year-old female patient came to me with three concerns: an unnatural smile caused by protruding upper teeth, a flat midface, and a mild prognathic (underbite) appearance. Her upper incisor axis was 122.3° — well above the normal range of 106–116°. Rather than traditional setback surgery, I planned a clockwise rotation of 4.2mm, which changes the tooth axis by approximately 2° per mm. The result? A 9° improvement in tooth axis, from 122° to 115°, with the underbite completely resolved.
To prevent a “monkey mouth” appearance (anterior fullness from rotation), I added 1mm maxillary setback and 1mm inferior repositioning. Since rotation makes the chin appear shorter, I also performed a genioplasty (chin advancement + lengthening) to complete the facial balance.
