✦ Initial Surgery Problems:
Excessive maxillary setback of 6.7mm + clockwise rotation
Mandible retracted approximately 14.73mm → extremely recessed chin
Airway narrowing → sleep-disordered breathing, frequent waking
Malocclusion + 1mm maxillary deviation to the right
Marionette lines + double chin developed
Patient unable to maintain normal social life

✦ Revision Surgery Plan:
Maxillary advancement: 6.5mm (restoring original position)
Counter-clockwise rotation (CCW): 2.5mm
Maxillary downgraft: 2mm (to increase upper tooth show)
1mm left shift of maxilla (asymmetry correction)
Mandibular advancement: ~11mm + genioplasty
Iliac bone graft to fill bone gaps

Nasolabial folds improved
Smooth jawline restored (no indentation)

⚠️ Key Lessons:
When the mouth goes back, the chin goes back too
Excessive rotation is the #1 reason for revision surgery
For males, a recessed chin is particularly detrimental
Balanced rotation and movement amounts are critical
