🚑 🚨 You're dispatched to a local urgent care facility for a 28-year-old female with an altered mental status. Dispatch advises the patient was found in the bathroom and appears to be in the 3rd trimester, approximately 8-9 months pregnant.
You find the patient in the bathroom hovering over the toilet. She is quite nauseated and has been vomiting clear liquid and food in the bathroom. The patient has an altered mental status. Her airway is patent, lungs are clear and equal, and her skin is warm and dry. However, you do notice that she has had urinary incontinence, which produces a strong urine ammonia odor. Assessing the abdomen, it is soft, and you don't notice any contractions. But you do notice that she has some slight pitting pedal edema.
Vitals:
• LOC: GCS 4-4-6
• HR: 98 normal sinus rhythm
• Eyes: PEARRL-A
• BP 182 / 102
• SPO2: 98%
• Temp: 98.6°F (37°C)
• Afib and slightly hyperglycemic
• BG: 146 mg/dL (8 mmol/L)
Medical staff provide you with a history chart:
• Hypertension and Type 2 DM
• Checked in for a "headache."
• Nurse states she was "fine when she got here."
