EMS is the second link in the STEMI Chain of Survival, and what happens in the field can change the entire trajectory of suspected acute coronary syndrome (ACS) / STEMI. In this lesson, we break down the key prehospital interventions EMS can perform before the patient arrives at the emergency department.
In this video, you’ll review how EMS providers:
✅ Perform rapid assessment, initial stabilization, and transport to an appropriate facility
✅ Support airway, breathing, and circulation (ABCs) while continuously monitoring vital signs
✅ Obtain a 12-lead ECG early to aid rapid recognition and downstream treatment decisions
✅ Stay prepared to start CPR immediately and defibrillate quickly if cardiac arrest occurs
We also cover common prehospital therapies often summarized as MONA:
▪️Oxygen when indicated, titrated to maintain SpO₂ at or above 94%
▪️Aspirin (commonly 160–325 mg) with attention to contraindications such as allergy or GI bleeding risk
▪️Nitroglycerin to improve perfusion and reduce chest discomfort, with key cautions (including hypotension risk and interaction concerns with erectile dysfunction medications)
▪️Morphine discussed for pain/anxiety relief and physiologic effects that can reduce cardiac workload
Finally, we highlight why a strong EMS-to-ED handoff is critical: clear reporting of the ECG findings, clinical status, and all interventions provided helps the ED team act faster during the most time-sensitive window.
