The primary assessment gets you through the immediate threats—but your next moves often depend on a solid secondary assessment. In this lesson, we break down a simple, repeatable workflow that helps you gather the right info fast and identify reversible causes early.
Secondary Assessment: The 3 Key Parts
1) Vital signs (first):
Start by obtaining (or re-checking) vitals—this is the foundation of most EMS and emergency algorithms and helps you trend deterioration quickly.
2) SAMPLE history:
✅ S – Signs & Symptoms: signs are measurable (RR, BP), symptoms are what the patient reports (SOB, chest pain).
✅ A – Allergies: medications, food, and environmental allergies.
✅ M – Medications: include prescribed meds, OTC, and herbal supplements.
✅ P – Past medical history: identify conditions that may be contributing (cardiac disease, asthma, diabetes, etc.).
✅ L – Last oral intake: critical for diabetics (insulin + no food) and important if surgery/anesthesia is possible.
✅ E – Events: what happened right before symptoms started—activity, rest, exertion, sleep, etc.
3) Hs & Ts (reversible causes):
After SAMPLE, run through the Hs & Ts—your “usual suspects” list—to spot a treatable cause behind the patient’s collapse or deterioration.
