Summary: Dr. Hetzler explains that sleep occurs in distinct stages measured by muscle tone, eye movement, and brain waves. A typical night cycles through non-REM stages (1-4) and REM sleep approximately every 90 minutes. Deep sleep (stage 4) occurs primarily early in the night and is crucial for brain restoration. During deep sleep, fluid-filled channels in the brain expand, allowing cerebrospinal fluid to wash away toxic metabolic products, including proteins linked to Alzheimer's disease. Poor sleep quality correlates with higher levels of these harmful proteins. After stroke, hypersomnia (excessive sleeping) is normal and beneficial—the healing brain requires 20% or more of total energy. Up to 78% of stroke survivors experience sleep problems, with sleep apnea affecting 50-70%. Three types of sleep apnea exist: obstructive (throat collapse), central (brain stops signaling breathing), and complex. Treatment options include CPAP machines and positional therapy. Stroke survivors should prioritize quiet, cool sleeping environments and consider cognitive behavioral therapy over medications when possible. Sleep tracking devices like Fitbit can help monitor deep sleep and REM cycles, providing valuable insights into sleep quality and recovery progress.
