The TBC@UBC Network was excited to host this online session on "Impact of Teamwork in Primary Care through HDC Discover Measures" in partnership with the Health Data Coalition (HDC) Team on Thursday, March 6, 2025, from 12 to 1:30pm.
The Health Data Coalition (HDC), a physician-led not-for-profit organization, provides clinical measures through its HDC Discover application. Built on a primary care data network, HDC Discover aggregates data from six Electronic Medical Record (EMR) systems across BC, offering a unique lens into practice patterns, patient panels, and health outcomes.
This session highlights three examples from across the province where team successes were recognized through clear, measurable impacts in the HDC Discover application. This type of feedback mechanism not only validates team efforts but also serves as a powerful motivator, reinforcing the connection between collaborative work and improved patient outcomes. We explore how real-time feedback supports teamwork—especially for developing teams—and why it is a critical component of continuous practice improvement.
Speakers:
Dr. Cole Stanley, Medical Director, Health Data Coalition
Dr. Cole Stanley is a Vancouver-based family physician with nine years’ experience practicing in BC. He received his MD from the University of Toronto and completed his Family Medicine Residency at the Jewish General Hospital, McGill University. Dr. Stanley is a clinical instructor with the UBC Faculty of Medicine and a family physician with the Hope to Health Research and Innovation Clinic in Vancouver’s Downtown Eastside.
Gayle Grout, Executive Director, Health Data Coalition
Gayle Grout is a seasoned Information Management leader with a passion to build software solutions to support our healthcare system in Canada. She has experience in IT Strategy, Software Implementation and Business Development and enjoys improving products, services, processes and supporting people development. Her ability to develop strategic plans and execute with discipline and structure creates an accountable, yet innovative, working environment for success. She applies her learnings, working to provide technical solutions for Canadian physicians to launch features that comply with regulatory requirements and enable physician workflows.
Cathy McGuinness, Manager, Community Engagement, Health Data Coalition
Cathy McGuinness is a member of the Clinical Services Team and brings over thirty years of experience in operations management, product development, business development and sales within the healthcare sector. As access to aggregate community data is a new concept, Cathy’s role is to support Divisions and Communities in understanding how to align priorities with community aggregate data, leverage Learning Programs with the use of community measures and drive change with quality improvement.
