A Bankart lesion is a tear of the shoulder labrum and capsule which often occurs when the shoulder is dislocated.
Surgery is usually needed and after surgery, rehabilitation is typically broken down into 3 phases: the immediate post operative phase, the intermediate post-operative phase and the advanced phase/return to work and sports phase. In this video, I demonstrate the exercises that I give my patients who are in the second phase, the intermediate post-operative phase.
In this phase of rehab, the movement and activity restrictions decrease significantly, we start to move the shoulder through more range of motion, and progress the strengthening portion of the program.
For the most part, patients are no longer restricted in their daily activities at this point, with a couple of exceptions. Reaching into abduction with external rotation of the shoulder must still be restricted. This might occur when reaching into the back seat of the car, or with throwing. This combination of movements stresses the repaired part of the shoulder too much. It isn’t until about 10-12 weeks post op that we would stretch the shoulder into this combination of movements, even then, only if movement is limited. Generally, aggressive stretching of the shoulder in most directions, but especially abduction + external rotation, needs to be avoided at this stage of recovery. An exception, is shoulder flexion, or reaching toward the front and up as if reaching into a cupboard. Gentle stretching into shoulder flexion is allowed and this allows patients to do most everyday tasks that require some reaching. The strengthening portion of the program progresses as well as we can now progress rotator cuff and scapular muscle strengthening a bit more aggressively. With that in mind, some exercises still need to be avoided. Push ups, bench press and chest flys for example can’t be done yet.
By the end of this phase of rehab, which takes us to the 12th post-op week, range of motion of the shoulder should be nearly normal, with the possible exception of external rotation. In addition, rotator cuff muscle and scapular muscle strength should be nearly normal.
