Just to be clear, you DO NOT need to get All Perfects to unlock the trophy. All you need is a Perfect Combo, which can be any combination of "Perfects" and Goods", so long as you don't get any "Bads". I just decided to torture myself to try and get All Perfects on each song. I won't be doing that again...

(There are no rewards for a sub-"C" rank, which is, in fact, a "D", or even a "C" rank, itself. Both are considered a "Fail".)
