There is a commen mistake in varicocele diagnosis. Although it is urged to do the varicocele doppler exam both in lying down and standing up positions in radiology guidelines most of the radiology centers apply the procedure in lying down position only. This makes a conflict between radiology and urology doctors. Especially in moderate varicocele cases although urology doctors find significant large veins in their physical examination the reports may come from radiologists below the expectation. This males a disbelieving among the urologists to doppler results and they choose to bellerive their physical examination is more accurate.

Our center is one of the most developed centers about varicocele surgery. We operate 200 cases per year in average. This made us expertise not only in operative techniques such as Istanbul Varicocelectomy procedure, but also in diagnostic evaluation. We recognized that the main difference between urologist and radiologist is the position of the patient during examination. So we begin to make our own doppler ultrasound and did it with the patient both in standing up and lying down positions. This detail changed our diagnostic quality extremely. Now we know that for high grade varicocele both position and even physical examination is enough for accurate diagnosis but for moderate grade varicocele standing up position enables us to see the real dimension of the enlarged veins. As people are living in upright position the examination and the ultrasound imaging must be done in this position.
