The following are the steps involved in setting up a ventilator which include: Program the machine to produce the required TV (Tidal Volume) (10 to 15 mL/kg). Set the machine to deliver the least amount of oxygen possible in order to maintain normal PaO2 (80 to 100 mm Hg). This setting may be set to a high value at first, but it will be gradually reduced based on ABG (Arterial Blood Gases) results.
Measure the maximum inspiratory pressure. Select the mode AC (Assist Control) or SIMV (Synchronized Intermittent Mandatory Ventilation) and rate as directed by the healthcare provider. If PEEP (Positive End-Expiratory Pressure) and PS (Pressure Support) are required, they should be set. Adjust the sensitivity so that the patient can activate the ventilator with little effort (usually 2 mm Hg Negative inspiratory force).
After 20 minutes of continuous mechanical ventilation, record minute volume and obtain ABGs to measure partial pressures of carbon dioxide, pH, and PaO2. Modify the settings (FiO2 and rate) based on the results of the ABG analysis to provide normal or healthcare provider-specified values. If the patient becomes confused or agitated suddenly, or begins bucking the ventilator for an unknown reason, assess for Hypoxia and manually ventilate on 100 percent oxygen with a resuscitation bag.
