Beyond The Grave (Anniversary Edition) - Area 24: Forgotten Chapel (100%)

Played on "Even Death Will Bleed", hardest difficulty. Originally, this was a secret level in a previous version of the game, however, this is now a regular level. In this level, there are lots of cultists and varied enemies, along with some powerful enemies here and there. Most of them are undead and cultists so most weapons will deal with them effectively. As for the stronger enemies like the Bishops and Arch Bishops, do be sure to use the Devil's Mule and Demon Eyes to take them out too. Also, in this level, there are lots of treasures to be found and it is easy to miss one or two (as seen near the end, when Sartana goes treasure hunting) due to missing a room or so in the chapel. Also at the end, there is a SECRET exit leading to the 2nd secret level, so don't miss it. Be aware of the serpents guarding the room to the secret exit though and use the hallways to your advantage in fighting them.
