The inquiry into the Manchester Arena bombing has found security failings on the night resulted in multiple missed opportunities to prevent the attack, saying it's highly likely more lives could have been saved.
The bombing killed 22 people and injured hundreds more when Salman Abedi detonated a device at the end of an Ariana Grande concert.
