Jack was a young man with advanced cancer who wished to live his last days of life at home, supported by his family. The team knew that his family were key to the delivery of this high quality care and supported them to lead the care he deserved.
Jack and his family were grateful to have a tailored, supportive and responsive service at such a critical time. He lived and died peacefully in a place where he wanted to be. Although it was distressing, the team approach and the dedication, courage and compassion of his family meant that the team achieved job and personal satisfaction second to none.
