We have extensively analyzed brain tumors.
For brain tumors, we have done the surgery and what is to be done after that is an important decision. The brain tumor is extracted and the cells are sent for biopsy. Pathologists, who are a different type of specialists. They use certain types of dyes in the cells and they provide a biopsy report stating what type of tissue it is. Biopsy is not just histopathology and up to many genetic tests are done using the tissue.

So what type of sample is it? Is it a good tumor or a bad tumor? Is it cancerous or will it grow again? We will get the answers to many such questions in the biopsy report. Once we get the report, we constitute a tumor board.

In the tumor board meeting we discuss every patient with the tumor board. Who would be part of the tumor board? Surgeons would be present, the radiologist who did the scan, the pathologist who analyzed the tissues, and the Medical oncologist, the radiation doctor. These 5 - 6 people collaborate and make a decision on how to stop further growth of the tumor that has been removed. The process does not stop with the surgery and the removal of the tumor alone. Follow up is very important. Ensuring that the tumor does not grow back is equally important.

If proper treatment is not given after surgery, there will be quick recurrence. It is the coordinated effort of a team. Dealing with brain tumors is not a single man show. It is a teamwork of a good neurosurgeon, a good radiologist, a good pathologist, a good anesthetist and subsequently, radiation oncologist, medical oncologist. a good team will ensure a good outcome for each patient.

As a surgeon I can briefly talk about the types of radiation. There are specialists for that, but if we see in general, earlier they used to give whole brain radiotherapy. Now that concept too has changed. You may ask, "Why should radiation be given after removing the tumor?" To ensure the tumor does not grow back in the same place, we give the treatment or adjuvant therapy. The radiation is meant to clear or sterilize the tumor bed. The radiation has to ensure that the tumor does not grow back.

Once the radiation therapy is given, at the same time, the medical oncologist gives tablets. Earlier they used to give injections for chemotherapy. For lung cancer and breast cancer, they give injections as part of systemic chemotherapy. There are a lot of complications for that. There would be a lot of hair fall, teeth can fall off, cell count will reduce, there could be bone marrow suppression. All these complications are a result of systemic chemotherapy.

In most cases, the chemotherapy given to the brain is very safe. They are only through tablets and not injections. Injections are very rare. Temozolomide is the most commonly given tablet for chemotherapy. It is relatively safe when compared to other chemotherapeutic agents. So for which patient, what type of therapy has to be given after surgery and how much radiation is to be given is discussed in the tumor board. Then we tell the patients
