Superior cluneal nerves come from the dorsal branches of L1 to L3. These nerves innervate the skin and the gluteal regions. The skin of the gluteus maximus and medius muscles. The superior cluneal nerve has three branches, medial, intermediate and lateral. The medial branch is confined with a tunnel consisting of thoracolumbar fascia. This tunnel could be a problematic area causing impingement of the nerve. Impingement of the nerve in this tunnel is similar to carpal tunnel syndrome in the wrist where the fascia constricts the median nerve.
When harvesting bone for bone grafting from the iliac crest, it is important to remember place extent of the superior cluneal nerves. The superior cluneal nerves can be injured if the incision exceeds 8 cm from the midline or 6 cm lateral to posterior superior iliac spine.
