Tip for Hospice Nurses: Medication Organization
At times you may have a patient whose pill planner always seems mixed up - no matter how carefully you fixed it, when you go back the pills are all in the wrong places. Even though we may never find out why it keeps happening, we need a process to ensure the patient is taking their meds as prescribed.
If you have access to a pharmacy that provides prefilled dose packs, this is the best option; however, if this is not an option, you can make your own dose packs as I describe in this video.
