Type Wound: Traumatic laceration
Appearance: wound approximately 11 cm x 3 cm irregular edges, 2 cm depth, moderate Serosanguinous drainage Surrounding skin pink, No erythema or warmth noted
oder: None detected
pain: 2/10 pt refused pain meds
treatment: area cleansed with normal saline dryed covered with non-adherent pad dressing secured with tape and label with data and time
Pt Response: pt tolerated Well
plan: continue to monitor wound for signs of Infection Change dressing every 24 hrs or as needed
