Mesenteric Ischemia Associated with Methamphetamine Use

In this educational radiology case, we review a contrast-enhanced CT of the abdomen and pelvis in a patient presenting with severe abdominal pain after recent methamphetamine use. The case demonstrates imaging findings consistent with mesenteric ischemia, including bowel wall thickening and areas of absent luminal enhancement.
We discuss the pathophysiology of stimulant-associated vascular complications, including catecholamine-mediated vasoconstriction and its effects on mesenteric circulation. The video highlights how mesenteric ischemia may occur even when major vessels such as the aorta and superior mesenteric artery appear patent, due to compromise of smaller mesenteric vessels.
This case also illustrates reperfusion injury and the imaging findings that may suggest bowel wall infarction.
