✍🏻 Documenting properly is kind of a big deal!

Think of it like this: if it’s not in the notes, it’s like it never happened 🤷‍♀️

When documenting, we need to be able to justify our skilled services all while making our notes make sense to the next person who reads them.

So, how do we decide what’s important vs what’s not to avoid being too wordy?

Some important details to include:

✅Skilled strategies you used (I.e 1 rep max, blocked practice, random practice)
✅Any education provided
✅Vital signs (pre-/post- treatment, more frequently for certain medical conditions)
✅Relevant communication with family members or other healthcare providers
✅Any equipment or device recommendations
✅Patient’s presentation and how they responded to treatment
✅Relevant subjective comments that led to the interventions facilitated in your session
✅Modifications/adaptations or progress

Examples of unnecessary details you can omit:

❌Every step performed in the intervention
❌Irrelevant social conversations
❌Personal opinions or judgments about the patient that are non-clinical
❌Avoid making assumptions about the patient’s thoughts or feelings unless they have been discussed and confirmed.
❌Excessive repetition

By doing this sorting act, we’re not only saving time but also making sure our records are clear and accurate 💯

It’s like decluttering your data - less mess, more focus.
