Описание: A 55 years old male diabetic, hypertensive Patient with past history of CABG 3 years back with patent graft on recent CT coronary angiography with progression of native vessel disease presents with crescendo angina for last 15 days. He has pulse of 88 mmHg with BP of 140 /90 mmHg. His Serum troponin I and CPK-MB are normal with ECG showing same old ST-T changes in anterior leads as noted 6 months back. His weight is 90 Kg and BMI of 30 Kg/m2. He was stable on Glycerine trinitrate 2.6 mg bd, metoprolol 100 mg 1 od, Aspirin 150 mg, 20 mg of atorvastatin hs and 40 mg of Telmisartan od. His recent HBA1C is 6.9 mg% with recent FBS/PPBS of 130/188 mg% on SU, Metformin, DPP4-I and Voglibose combination daily. Her recent Echocardiography shows LVEF – 48% with LVDD/DS- 48/28 mm with grade III diastolic dysfunction. His last NT-pro-BNP was 860pg/dl.
