The National Transportation Safety Board (NTSB) has concluded that a single loose wire was a critical factor in the sequence of events that led to the container ship Dali striking and collapsing the Francis Scott Key Bridge in Baltimore.
NTSB determined that wire 1, which was not properly seated in a circuit breaker terminal block 381, likely triggered the initial power outage on the vessel.

During a public meeting, NTSB investigators explained that the loose connection in the ship's electrical system caused a breaker to open unexpectedly. This initiated a chain reaction, resulting in two blackouts that left the Dali without propulsion and steering moments before it collided with the bridge on March 26, 2024.
The tragic incident resulted in the loss of six construction workers who were on the bridge at the time. NTSB Chair Jennifer Homendy stated, "Finding this single wire was like hunting for a loose rivet on the Eiffel Tower."
Investigators found that a wire-label banding prevented the wire from being fully inserted into a terminal block, resulting in an inadequate connection.
The investigation also highlighted that the bridge's vulnerability to collapse from a vessel impact was a contributing factor to the severity of the disaster. The NTSB noted a lack of countermeasures that could have mitigated the risk posed by large vessels, which have grown in size since the bridge was built.
