Early pregnancy ultrasound shows a gestational age of about 9 weeks by dates, with an embryo measuring only around 7 weeks, indicating growth lag. There is absence of fetal cardiac activity and fetal movements, which are key indicators of non-viability at this stage. The yolk sac appears echogenic, an abnormal feature strongly associated with poor pregnancy outcome and embryonic demise. A subchorionic hematoma measuring approximately 10 × 15 mm is seen adjacent to the gestational sac, representing perigestational bleeding. The combination of growth discrepancy, absent cardiac activity, abnormal yolk sac, and associated subchorionic hematoma is diagnostic of early pregnancy failure, most consistent with a missed abortion. Overall prognosis is poor, and management includes confirmation and appropriate medical, expectant, or surgical treatment.
