Verapamil is a calcium channel blocker primarily affecting cardiac and vascular smooth muscle. It works by inhibiting L-type calcium channels, which are crucial for calcium influx during action potentials. In cardiac muscle, this leads to decreased contractility (negative inotropic effect) and reduced heart rate (negative chronotropic effect) by slowing conduction through the AV node. However, skeletal muscle does not rely on extracellular calcium for contraction; instead, it uses calcium released from the sarcoplasmic reticulum in response to action potentials. Therefore, verapamil has minimal effect on skeletal muscle contraction.
