In this closing keynote, Don Berwick discusses the impact and significance of the UK Staffordshire enquiry into hospital mortality. In this he presents his perspective on how these findings may not be unique and indeed warns how such failings may be more widespread among other healthcare organisations and systems. He also discusses how such lessons can be addressed and the courage and challenges that this involves.
