Why is diagnosing severe AR still such a challenge? And even when diagnosed, why are only 25–30% of these patients receiving surgical aortic valve replacement? The discussion highlights the need to distinguish between younger and older patients—while mechanical valve replacement is an option for younger individuals, treatment options for elderly patients remain limited.
