Dr. Shapiro does a lot of rhinoplasties. It's one of his favorite procedures and people come in and kind of want to know whether they're a good candidate for a rhinoplasty and oftentimes the question is, "Who is not a good candidate for a rhinoplasty?" He thinks it's very important to understand that when he is seen in consultation, not only is the patient interviewing him, he’s also interviewing the patient. What he’s looking for is a very healthy state of mind and make sure emotionally that the patient is motivated in the correct fashion for having elective cosmetic surgery.

When we see somebody we try to figure out technically whether we can make them happy which means that if we can accomplish aesthetically what we both are trying to decide to do to perform in terms of the operation. Then we try to figure out whether or not emotionally the patient is ready for it and what their motivation is. For example, if in fact we see somebody that has an idea of what they want but basically is not something that we think can be accomplished technically in a very predictable fashion, we would say that either they're not a candidate for the rhinoplasty or we are not the surgeon that should perform that rhinoplasty.

We have a very, very low, almost zero revision rate and we try to pick our patients accordingly. Another question in terms of who is a good candidate is we think that when we see somebody who is an ethnic rhinoplasty if you will, that has an ethnic background, we want to make sure that we're on the same page in terms of what they're looking for. We like to preserve the ethnicity and there are some patients that really need to rectify with themselves and their family what they're trying to accomplish. There are some families that give some kind of resistance to patients getting a rhinoplasty because they want to maintain their ethnicity and they find it troublesome if somebody wants to evade basically their roots if you will.

In terms of who is a good candidate and not a good candidate for a rhinoplasty. Number one, we make sure that the patients are motivated correctly, that emotionally they're mature and that they're not looking for secondary gains. If somebody thinks that a rhinoplasty is going to completely change their life, we think they're mistaken. We think that it will greatly enhance somebody's life in terms of both emotionally and aesthetically and we see such an increase in the patient's confidence after a rhinoplasty but it certainly won't get them in a job or keep them in a relationship or all of a sudden get a relationship. We try to screen accordingly.

Also we think there's a whole process that we go through in terms of imaging photographs and we like to have patients bring in photographs of noses that they like. If we can't match minds in terms of coming up with a common aesthetic plan, again, we’re not the surgeons and we don't think they're a good candidate for us to perform the rhinoplasty.
