The Balance Error Scoring System (BESS) is one of the most widely used orthopedic tests for assessing static postural stability. In this video, Dr. Donald Ozello, DC of Championship Chiropractic and CCEDseminars, breaks down the complete protocol used for evaluating concussions, proprioception, and vestibular function.
The BESS Protocol (6 Conditions): Each position is performed with eyes closed for 20 seconds.
Double Leg Stance (Firm Surface)
Single Leg Stance (Non-Dominant Leg, Firm Surface)
Tandem Stance (Non-Dominant Leg in Back, Firm Surface)
Double Leg Stance (Foam Surface)
Single Leg Stance (Foam Surface)
Tandem Stance (Foam Surface)
Scoring Errors (1 Point Each):
Opening eyes.
Lifting hands off iliac crests (hips).
Stepping, stumbling, or falling.
Moving hip into more than 30 degrees of abduction/flexion.
Lifting forefoot or heel.
Remaining out of test position for more than 5 seconds.
