A spherical left ventricle represents pathologic remodeling, where ventricular geometry shifts from its normal ellipsoid form to a more spherical shape. increasing sphericity predicts heart failure hospitalization and sudden cardiac death, independent of ejection fraction.
The sphericity index is calculated as the ratio of short-axis diameter to long-axis length in the apical four-chamber view. Normal values are around 0.5 to 0.6; values above 0.7 are pathologic.
