Our next series of videos will focus on pediatric vitreoretinal surgery featuring Dr. Aaron Nagiel who is the Chief of Pediatric Retina at Children's Hospital Los Angeles. This case is a 13 year old patient with a history of FEVR complicated by progressive exudative and tractional retinal detachment encroaching on the macula. The patient has been treated with laser photocoagulation and intravitreal anti-vegf injections and the goal of surgery is to relieve vitreoretinal traction and to decrease any tractional component that is encouraging vascular leakage. The patient is managed with a combination of a scleral buckle and vitrectomy with techniques that are applicable to a wide variety of peripheral tractional retinal detachment etiologies. At the end of the case we discuss some of the clinical manifestations of FEVR and review the staging system that can help guide management.
