The Neurodevelopmental Disorders Care Coordination project was designed to address identified gaps by providing consistent and formal coordination support for the families of children and youth with concurrent complex neurodevelopmental, medical and mental health needs. The project aims to improve health service delivery, and ultimately outcomes, for this population through enhanced family supports and care coordination that will better address their interrelated health, education and social needs.
We will share what we are learning through the project, as well as introduce you to some of our families via case study and an invited panelist.
