There are three checkboxes on the form for initial exemption, amending the form, and revoking the exemption.
An amendment is required anytime the information on the original designation of exempt person becomes outdated or changes, such as a change in the customer's legal name, address, tax ID number, type of exemption, ownership structure, or business structure.
An amendment is not required for every annual review or for temporary operational changes.
Formally revoking the designation of exempt person is optional, as the entity can just start filing CTRs instead. However, many organizations choose to file the revocation for internal business practices.
