Samreen Ahmed, FRCP, MD, MBBS, MSc, University Hospitals of Leicester NHS Trust, Leicester, UK, discusses the mechanisms of resistance to antibody-drug conjugates (ADCs) and how these insights have driven drug development. She explains the importance of ADCs in delivering potent chemotherapy directly into cancer cells and highlights resistance mechanisms, including receptor mutations, loss of target receptors, and challenges in ADC internalization. Advances such as cleavable linkers and the bystander effect have significantly improved ADC efficacy, allowing chemotherapy to impact even heterogeneous tumors. This interview took place at the BTOG 2025 congress in Belfast, UK.
