Questions to Ask:
Ask the group the following questions:
• What safeguards (controls or barriers) are used to manage these hazards in our workplace?
• How could complexity in the workplace lead to similar incidents in our workplace?
• When would a hazard assessment typically be conducted, before or after the wheel chocks are in place?
• How do we prevent and mitigate snow and ice as a potential dropped object?
• Do our exclusion zones have capacity to handle a deflection?
• What Potentially Serious Incidents (PSI) involving dropped objects have we had?
• Do our safeguards have capacity to fail safely if an error or mistake is made?
