Available for the first time in rural areas, small, and midsize cities, PLACES (Population Level Analysis and Community Estimates) provides 27 model-based chronic disease related measures — including health outcomes (e.g., heart disease, COPD), unhealthy behaviors (e.g., current smoking, physical inactivity), and preventive service use (e.g., annual checkup, mammography use) — for which data can be accessed down to a local level. These data can be used to identify geographic health disparities, establish health goals, and target programs and policies where people live.
