Choosing the right IV fluid is a daily clinical decision. Here’s a rapid refresher on the most common indications to keep your practice sharp.
✅ Normal Saline (NS):
Vomiting (replaces chloride loss)
Initial resuscitation in DKA
Severe hyponatremia (use 3% NS)
Hypochloremic alkalosis
✅ Ringer's Lactate (RL):
Diarrhea & Dehydration
Burns
Pancreatitis
Initial trauma & hemorrhage resuscitation
✅ Half Normal Saline (0.45% NS):
Hypernatremic dehydration (e.g., from fever/sweating)
Maintenance fluid
Gradual correction of hypernatremia
✅ Albumin:
Hepatorenal Syndrome (HRS)
Spontaneous Bacterial Peritonitis (SBP)
Large volume paracentesis in cirrhosis
✅ IV Dextrose:
Hypoglycemia (Adult & Neonatal)

Hyperkalemia (as part of a Glucose-Insulin drip)
