In 2008, James Titcombe lost his baby son Joshua following poor care at the Morecambe Bay Trust. This tragic event led to a national inquiry and major changes to improve the safety of maternity care in England.
As well as sharing his own story, in this talk, James discusses topics such as; the importance of learning from things that go wrong in healthcare, the direction of change in patient safety within the NHS, the importance of culture and leadership, and the difference individual stories can make in influencing and driving positive change.
