Cerebellar Physical Exam for Royal College
Gait Examination:
wide-based ataxic gait. sway with “drunken gait”
Stagger & reel in all directions.
Heel-to-toe walking more sensitive.
Romberg – unable to keep balance with eyes both open & closed
Truncal ataxia
Head & Neck Examination
: Speech
slurred with irregular volume & rhythm. Conjugate horizontal jerk nystagmus on lateral gaze not suppressed with retinal fixation
and rebound nystagmus.
Extremities: Intention tremor that is large amplitude
.
Finger-to-nose testing
for dysmetria. Uncoordinated rapid alternating movements (dysdiadochokinesia). Heel-to-shin testing. ↓ Muscle tone.
Reflexes: Pendular patellar reflex.
Rebound phenomenon.
