What is population health?

Описание: Population health management programs bring together existing but disconnected data to reveal health risks and find opportunities to reduce those risks. These programs take many forms, but one common feature is that they focus on a common goal across a large population, such as reducing the risks of heart disease.

For Population Health to be successful, having the right treatments is important. But even more important is the collective strength that occurs when health systems, governments, companies, non-profits and others work together around a shared goal.
