Here are the ten areas for you to evaluate to help determine if it's time to make the move.

1. Can they still use the phone, dial 911?
Are they calling for the same reason over and over?
2. Can they still drive or arrange transportation?
3. Eating nutritious meals?
4. Is the home clean and sanitary?
5. Are they taking medications as directed by their doctor?
6. Are they a fall risk?
7. Have they forgotten to turn off the stove or water?
8. Do they need help with personal hygiene?
9. Are they socializing with others?
10. Are they staying hydrated?
