Patients who have facial paralysis or Bell's palsy often have several concerns, including:
-Asymmetrical facial appearance
-Functional issues with eyes and mouth
-Difficulty speaking
-Difficulty chewing
GOALS of facial reanimation surgery therefore include:
1. Functionality - need to make sure eyes close properly so they are protected; help reduce drooling and improve articulation by changing the position of the corners of the mouth.
2. Improved symmetry and create the ability to spontaneously smile.
Ideally we want the patient to have to ability to smile spontaneously, so they don't have to think about it. So when appropriate, Dr. Azizzadeh will recommend treatment that spontaneously improves the smile.
The most common treatment to achieve a spontaneous smile is Selective Neurolysis. This type of surgery is appropriate for patients with some facial movement. However, if the patient does not currently have any movement in the paralyzed side of their face, Dr. Azizzadeh will recommend cross face nerve graft with gracilis muscle transfer.
