There are 2 different major approaches when it comes to Agile at scale - in terms of team structures:
1. Agile Train
2. Agile Tribe
Agile Train is nothing but the formation of a Virtual Team of Real Teams whereas Tribe is the Real Team of teams. It has the same structure as Organization's structure.
Both of them can be successful or both can fail. Success or failure depends on the context in which you are going to choose the approach.
