Decoding Cardiac Markers in MI!
When a patient presents with suspected Myocardial Infarction, cardiac markers are our go-to for accurate diagnosis and management. Here's a quick recap of the essentials:
🔑 Key Markers & Their Role:
Troponin T & I: The gold standard! Most sensitive and specific markers for myocardial injury. They typically rise 3-6 hours post-MI and can remain elevated for days.
Myoglobin: The earliest non-specific marker. It rises quickly (within 1-4 hours) but also clears rapidly, making it useful for early detection, but less specific due to presence in skeletal muscle.
CK-MB: Crucial for detecting *re-infarction*. While Troponins remain elevated for longer, a new rise in CK-MB after an initial MI episode can indicate a fresh event.
LDH (Lactate Dehydrogenase):Look out for the "flipping pattern" (LDH1 more then LDH2)! While not as specific as troponins, this can be an indicator of MI, especially in older cases where troponins might be declining.
