POCUS - RUSH protocol in shock patients:
The "RUSH" protocol is used to quickly assess any patient with undifferentiated shock and hypotension. The HI-MAP mnemonic is used to guide the user through the required views of the RUSH protocol.
"RUSH" stands for "Rapid Ultrasound for Shock and Hypotension".
"HI-MAP" stands for: Heart, Inferior vena cava, Morison's pouch, Aorta, and Pneumothorax.
Heart assessment includes four views: parasternal long, parasternal short, apical four chamber, and subxiphoid. The three main elements to assess for include left ventricular contractility, right ventricular dysfunction, and pericardial effusion with or without evidence of tamponade.
Inferior vena cava assessment evaluates volume status. A distended IVC suggests hypervolemia, while a collapsed IVC suggests hypovolemia. The degree of IVC size change during respiratory cycle can be used to predict volume responsiveness.
Morrison's pouch assessment includes e-FAST exam. E-FAST stands for Extended Focused Assessment with Sonography in Trauma.
Aorta assessment evaluates for abdominal aortic aneurysm or aortic dissection.
Pneumothorax is confirmed by the absence of lung sliding and presence of lung point sign.
