High-income physicians don’t usually have a spending problem — they have a reimbursement and documentation problem.
In this workshop, our Director of Tax, Rob Bolduc, EA, and I break down the Accountable Plan strategy and how physician-owned practices can legally turn business expenses into tax-free reimbursements.

Who This Video Is For
If you’re a self-employed physician or medical practice owner paying for CME travel, a home office, phone, mileage, or professional resources out of pocket, this video is for you.
This is especially relevant if your practice is profitable, but your personal accounts are still covering expenses that should be reimbursed through the business.
Why Physicians Still Pay Taxes on Deductible Expenses
Many practice owners pay legitimate business-related costs personally, then never get reimbursed by the practice.
The result is simple: you end up using after-tax dollars for practice expenses, and the deduction often gets missed or diluted — which quietly increases your effective tax bill year after year.
The Accountable Plan Strategy
An Accountable Plan is an IRS-approved reimbursement arrangement that allows your practice to reimburse you for qualified business expenses in a structured way.
When it’s set up correctly, the reimbursements are tax-free to you and tax-deductible to the business, under Treasury Regulation §1.62-2.
What You’ll Learn in This Video
In this workshop, you’ll learn:
What an Accountable Plan is and why it matters for physician-owned practices
Which common physician expenses can often be reimbursed tax-free when properly documented
The three IRS requirements that determine whether reimbursements stay compliant
How the step-by-step process works, including timing, receipts, and expense reports
How Accountable Plans fit into a broader tax strategy alongside compensation and retirement planning
A Practical Example
Rob walks through an illustrative physician example involving a practice owner structured as an S-Corp who reimbursed roughly $30,000 per year in qualified mixed-use expenses.
The example shows how proper reimbursement structure and documentation can translate into meaningful annual tax savings without increasing spending or workload.
When This Strategy Makes Sense (And When It Doesn’t)
Accountable Plans tend to make sense when you have consistent, legitimate business expenses that are currently being paid personally, and your practice has clean bookkeeping and a separate business bank account.
They may not be a fit if expenses aren’t well documented, reimbursement habits are inconsistent, or the practice isn’t prepared to follow the substantiation and timing requirements that keep the plan audit-ready.
This is also not a “set it and forget it” strategy — it requires a written plan, receipts and logs, timely expense reporting, and returning any excess reimbursements when applicable.
Next Steps for Physicians
If you’re a self-employed physician or medical practice owner earning strong income and paying $10,000+ per year of practice-related costs personally, this is a strategy worth understanding.
