Department Of Labor forms, The CA-16: This for is an authorization for examination and/or treatment. This form authorizes an injured employee to obtain examination and treatment for up to 60 days. This forms are issued by the ICCO or trained personnel. This form must be issued within 4 hours in traumatic injuries requiring medical attention.
