The federal investigation into the fatal UPS 2976 crash in Louisville revealed that a major defect in the failed engine was already known to Boeing's service team, but Boeing did not believe the issue would create a serious flight safety risk.

In its latest update on the crash of UPS Flight 2976, the National Transportation Safety Board (NTSB) said investigators found widespread fatigue cracking and structural failure in the bearing race, the part that connects the left engine to the wing.
After analyzing Boeing's service records, the NTSB confirmed that the failed bearing matched the original design. However, a Boeing Service Letter from February 7, 2011, showed that Boeing already knew about four similar bearing failures on three different aircraft. In those cases, the bearing race cracked, split apart, and shifted out of position. Even so, Boeing told operators at the time that the issue was not expected to create a "safety of flight" problem.
Boeing later added inspections of this bearing assembly to routine MD-11 maintenance, scheduled every 60 months.
Although Boeing discussed a redesigned bearing assembly, operators were still allowed to keep using the original version.
Because of these findings, the NTSB says it is now reviewing what actions should be taken next as part of the ongoing investigation.
