This interview with Dr. Steve Lambert highlights key insights into sleep disordered breathing (SDB) and obstructive sleep apnea (OSA) with special focus on 𝐧𝐨𝐧-𝐚𝐧𝐚𝐭𝐨𝐦𝐢𝐜𝐚𝐥 𝐟𝐚𝐜𝐭𝐨𝐫𝐬 such as 𝐢𝐦𝐩𝐚𝐢𝐫𝐞𝐝 𝐩𝐡𝐚𝐫𝐲𝐧𝐠𝐞𝐚𝐥 𝐝𝐢𝐥𝐚𝐭𝐨𝐫 𝐦𝐮𝐬𝐜𝐥𝐞 𝐟𝐮𝐧𝐜𝐭𝐢𝐨𝐧, 𝐥𝐨𝐰 𝐚𝐫𝐨𝐮𝐬𝐚𝐥 𝐭𝐡𝐫𝐞𝐬𝐡𝐨𝐥𝐝, and 𝐮𝐧𝐬𝐭𝐚𝐛𝐥𝐞 𝐯𝐞𝐧𝐭𝐢𝐥𝐚𝐭𝐨𝐫𝐲 𝐜𝐨𝐧𝐭𝐫𝐨𝐥 (𝐡𝐢𝐠𝐡 𝐥𝐨𝐨𝐩 𝐠𝐚𝐢𝐧).
🔑𝐒𝐨𝐦𝐞 𝐊𝐞𝐲 𝐏𝐨𝐢𝐧𝐭𝐬🔑:
✅ Nearly all dental problems are related to 𝐚𝐢𝐫𝐰𝐚𝐲 𝐜𝐨𝐦𝐩𝐞𝐧𝐬𝐚𝐭𝐢𝐨𝐧𝐬 🦷.
✅ OSA involves both 𝐚𝐧𝐚𝐭𝐨𝐦𝐢𝐜𝐚𝐥 (𝐬𝐭𝐫𝐮𝐜𝐭𝐮𝐫𝐚𝐥) 𝐚𝐧𝐝 𝐧𝐨𝐧-𝐚𝐧𝐚𝐭𝐨𝐦𝐢𝐜𝐚𝐥 (𝐟𝐮𝐧𝐜𝐭𝐢𝐨𝐧𝐚𝐥/𝐛𝐞𝐡𝐚𝐯𝐢𝐨𝐫𝐚𝐥) components, like impaired pharyngeal dilator muscle function, low arousal threshold, and unstable ventilatory control (high loop gain) 🌬️.
✅ The goal of treatment is to achieve a "𝐜𝐫𝐢𝐭𝐢𝐜𝐚𝐥 𝐜𝐥𝐨𝐬𝐢𝐧𝐠 𝐩𝐫𝐞𝐬𝐬𝐮𝐫𝐞" (𝐏𝐜𝐫𝐢𝐭) of minus 10 centimeters of water, meaning the airway withstands negative pressure without collapsing 🎯.
✅ 𝐎𝐫𝐚𝐥 𝐚𝐩𝐩𝐥𝐢𝐚𝐧𝐜𝐞𝐬 are management tools, not cures, and although they can be very effective for OSA, they usually cause bite changes 💡.
✅ Effective treatment requires a comprehensive " 𝐚𝐢𝐫𝐰𝐚𝐲 𝐛𝐮𝐟𝐟𝐞𝐭 " approach, combining multiple therapies rather than monotherapy, addressing structure, function, and behavior. 🧠
✅ Dr. Lamberg wrote the book 𝑻𝒓𝒆𝒂𝒕 𝒕𝒉𝒆 𝑪𝒂𝒖𝒔𝒆... 𝑻𝒓𝒆𝒂𝒕 𝒕𝒉𝒆 𝑨𝒊𝒓𝒘𝒂𝒚: 𝑻𝒉𝒆 𝑹𝒐𝒍𝒆 𝒐𝒇 𝑺𝒏𝒐𝒓𝒊𝒏𝒈 𝒂𝒏𝒅 𝑺𝒍𝒆𝒆𝒑 𝑨𝒑𝒏𝒆𝒂 𝒊𝒏 𝑪𝒐𝒏𝒕𝒆𝒎𝒑𝒐𝒓𝒂𝒓𝒚 𝑷𝒓𝒆𝒗𝒆𝒏𝒕𝒊𝒗𝒆 𝑴𝒆𝒅𝒊𝒄𝒊𝒏𝒆, developed the 𝐋𝐚𝐦𝐛𝐞𝐫𝐠 𝐏𝐫𝐨𝐭𝐨𝐜𝐨𝐥 for screening, diagnosis, data collection, and targeted treatment, and the 𝐋𝐚𝐦𝐛𝐞𝐫𝐠 𝐐𝐮𝐞𝐬𝐭𝐢𝐨𝐧𝐧𝐚𝐢𝐫𝐞 as a risk assessment tool 📋. He also created the 𝐋𝐚𝐦𝐛𝐞𝐫𝐠 𝐒𝐥𝐞𝐞𝐩 𝐖𝐞𝐥𝐥 𝐀𝐩𝐩𝐥𝐢𝐚𝐧𝐜𝐞.

Dr. Steve Lamberg has been practicing all phases of general dentistry in Northport, New York, for 40 years with a focus on comprehensive restorative dentistry. He is a Diplomate of the American Board of Dental Sleep Medicine and has served on their board review faculty. He is the inventor of the Lamberg SleepWell Appliance “LSW”, which is an FDA-cleared appliance for the treatment of OSA and holds several related patents. He regularly contributes articles to peer-reviewed journals, is on the editorial review board of Dental Sleep Practice, and lectures internationally to help dentists integrate the many aspects of dental sleep medicine into their general dental practices. Dr. Lamberg also launched, and serves as the director of, the Pediatric and Adult Airway Network of New York (PAANNY), to provide a regional platform where dentists, physicians, orofacial myologists, and other related professionals learn and collaborate on treatment patients of all ages. Additionally, Dr. Lamberg serves as a Scientific Advisor for airway at the Kois Center in Seattle and has created a course there focusing on practicing dentistry on airway compromised patients. Dr. Lamberg also created and teaches a curriculum for Nierman Dental Sleep Medicine mini-residency. After creating a medical screening questionnaire, the Lamberg Questionnaire, he went on to publish a supporting book, "Treat the Cause…Treat the Airway”.
